Donating is easy…

* You can donate through payroll deduction. Payroll deduction donations are easy to set up! Just three simple steps:

1. Fill out state controllers form and print it  - http://e-advancement.csupomona.edu/forms/cd88_fill.pdf  Follow instructions on the second page on how to fill it out.

2. Print payroll deduction request worksheet and fill it out - http://e-advancement.csupomona.edu/forms/Payroll%20Deduction%20Request%20Worksheet.pdf.
(Division=Student Affairs, Dpt.=leave blank, Account Name=LFSSA, Account#=414700, Dollar Amount=your monthly donation amount; sum your grand total; sign and date)

 3. Walk your donation forms to Dr. Rebecca Gutierrez Keeton, Dean of Students, bldg. 26, rm. 110, for LFSSA tracking. Rebecca will then have your donation processed through Gift Processing in University Advancement. * You can also do a one-time tax deductible donation if you prefer! Make your check payable to: Cal Poly Pomona - LFSSA. Write down our account number on the check: 414700. Send check to Rebecca Gutierrez Keeton for LFSSA tracking to bldg. 26, rm. 110, in a sealed envelope. Rebecca will then have your donation processed through Gift Processing in University Advancement.
