Full Day State Program (Free or Reduced Fee)

Please complete the following forms below. Once completed please call our offices at (909) 869-2284, to make an enrollment appointment. Enrollment appointments are about one hour long and parents must bring the child's original Birth Certificate and Immunization card. Parents are also asked that they bring three consecutive pay stubs for all adults counted in the family size. In addition to the pay stubs, each adult counted on the family size must complete and return either a training vertification, employment verification or statement of incapacity form to be kept on file at the Children's Center.
