ACCEPTING STAR NOMINATIONS FOR:

Summer QUARTER 2009

* deadline is Thursday, August 13th by 6 p.m. *

 STAR awardees positively impact the lives of Cal Poly Pomona students and enhance the Division of Student Affairs through their service to the campus community.  Please nominate a staff member (sorry, no MPP’s) who, by their demonstration of the Division of Student Affairs Mission and Values, deserves this special recognition by completing the following information.  Referencing the Student Affairs Mission Statement - “To cultivate student learning and success through student-centered values and actions” and the Student Affairs Values listed below, please indicate which of these your nominee exemplifies, and how, by the specific examples you include in the sections below.  To assist you, expanded definitions of the Mission and Values are included at the Division website: http://www.dsa.csupomona.edu/division/mission.asp

Peer and Self nominations are welcomed. Up to four STAR Employees will be selected each quarter. 

You are eligible to be nominated for a STAR Award if:
 -you are a State, ASI, or Foundation employee
 -you have one year of employment
 -haven't received a STAR Award within the last five years
 -you have received a STAR Award within the last five years, you must be in a new role/position to be eligible
