Vendor Program - Items needed prior to arrival

Please send back the following items at least 2 weeks prior to the scheduled event:

1.         A signed Vendor Contract (page #2)

2.         Completed New Vendor Registration Form

3.         Completed Commercial Permit

4.         A check/money order made payable to the Cal Poly University, Office of Student Life. 5.         A current California Resale License or Federal ID Number document

6.         An insurance certificate and an endorsement in the amount not less than $1,000,000 for injuries, including accidental death, to any one person, and subject to the same

minimum for each person, in the amount not less than $1,000,000 for each

accident and property damage insurance in an amount not less that $500,000.

 

The insurance carrier must have an overall rating of “A” or better with a financial

rating of “7” or better.

 

The University must be listed as an additional insured and the language must

read as follows:

 

“That the State of California, the Trustees of the California State University, the

 University and the employees, officers and agents of each of them are

 included as additional insured.”
